WHAT IS IT?

Soft tissue masses in the foot and ankle include a variety of benign and, rarely, malignant growths arising from the skin, fat, tendons, nerve sheaths, blood vessels, and joint linings. The most common are ganglion cysts, lipomas, and plantar fibromas. Though usually benign, all new or changing masses should be properly evaluated.

No — ganglion cysts are benign, fluid-filled sacs attached to a joint or tendon sheath. They are not cancerous. However, they can cause pain and should be treated if they compress a nerve or become symptomatic. Some disappear on their own; others require aspiration or surgical removal.
A plantar fibroma is a benign fibrous nodule growing within the plantar fascia on the bottom of the foot. It typically feels like a firm, rubbery lump in the arch. Unlike ganglion cysts, plantar fibromas cannot be aspirated. Treatment includes corticosteroid injections, orthotics, and occasionally surgical excision
Most foot masses are benign, but characteristics that warrant urgent evaluation include: rapid growth, hard immobile consistency, size greater than 5 cm, deep location, and associated skin changes. We evaluate all masses and refer appropriately when any concern for malignancy exists.
For small, asymptomatic masses with clearly benign characteristics (e.g., a small ganglion cyst), watchful waiting is a reasonable option. Any mass causing pain, growing, or of uncertain nature should be evaluated promptly.
After aspiration, the cyst typically collapses and the pain resolves. Recurrence is possible (roughly 50% recur after aspiration alone). Surgical excision removes the cyst and its root, providing more durable resolution with lower recurrence rates.
